In accordance with Texas House Bill 4224 (89th Regular Session) and Section 181.105 of the Texas Health and Safety Code, the following information is provided to assist consumers in accessing health records and submitting questions or complaints.
Requesting Your Health Care Records
Clients may request copies of their health care records by submitting a written request directly to Holistic Purpose, PLLC.
Requests should include:

A description of the records requested

Your preferred method of delivery
Requests will be processed in accordance with applicable Texas and federal laws governing access to medical records.
